In order to facilitate smooth operations through the program several forms and manuals are provided. These forms are unique for SPS. Forms from other WJU programs including the traditional undergraduate program should not be used.
Any change in one’s program requires written authorization from the student. Occasionally, authorization is required from both the student and the current instructor. Forms for these changes are listed below.
